Birth Certificates

While in the hospital, you will be given a birth certificate worksheet from the South Carolina Department of Health and Environmental Control (DHEC). This worksheet is used to gather the information required for your baby's birth certificate, such as whether or not you want RMC to apply for your baby's Social Security number. Before you are discharged from the hospital, you will be visited by an RMC Health Information Management (medical records) employee, who will pick up your completed DHEC worksheet and answer any questions.

  • What are the benefits of establishing paternity?
Both parents and the child have the right to a full parent-child relationship. Both parents deserve an opportunity to develop, enjoy and grow in this relationship. The father has the right to know the child and to contribute to the success of the child's future. The greatest statistical indicator of whether or not an unmarried dad will be involved in his child’s life is whether or not he establishes paternity.
  • What do I need to take to the hospital to establish paternity for my child?
An unmarried mother and father must show valid, government-issued, picture identification (passport, driver's license, state identification card) in order to sign the Affidavit of Paternity. Expired identification is not acceptable. A state identification card can be obtained at a local Motor Vehicles Office for a small fee.
