from the South Carolina Department of Health and Environmental Control
(DHEC). This worksheet is used to gather the information required for
your baby's birth certificate, such as whether or not you want RMC to
apply for your baby's Social Security number. Before you are discharged
from the hospital, you will be visited by an RMC Health Information Management
(medical records) employee, who will pick up your completed DHEC worksheet
and answer any questions.
RMC cannot issue birth certificates. You may obtain your child's birth
certificate from the Vital Records Office in person or by writing to:

You may also obtain the birth certificate application by visiting www.scdhec.gov (go to Vital Records and click on Birth Certificate).
The Vital Records Office will ask that you wait two to three weeks after your baby's birth before requesting the birth certificate. It is very important to purchase your child's birth certificate before his/her first birthday.
If you have questions, please contact the State Vital Records Office at 803-898-3630.
