Referrals/Admissions
A free pre-admission screening is provided to all referred patients by one of the members of the rehabilitation team. The Medical Director is responsible for authorizing a patient’s admission into the program.
In order to provide prompt service, the following information is helpful to provide:
- Patient’s name, home address, date of birth and social security number
- Name of the patient’s referring or family physician
- History of present illness
- Insurance information
The RehabCentre accepts most major medical insurance carriers, including Medicare.
